New Water Service Installation Agreement

I, the undersigned, request installation of a new water service for the address entered below. For normal residential water demand this will include a maximum of 60’ of ¾ inch K Copper water line from the water main to the meter setting at property line, 5/8” meter, meter connections, meter box and cover. When directional boring, street and sidewalk repairs, or other extraordinary work and materials are needed, I understand and additional charge will be assessed for the cost of the additional work and materials.

I further understand that my request will be placed on a chronological list installation will occur as soon as possible (normally within 7 to 10 days).

It is my responsibility to determine the elevation of the final grade in the area where the water line and meter are to be installed. I will notify both LWD and my plumbing contractor of the final grade so the water line and meter can be installed at the correct depth. If necessary, I will have the site staked to identify final grade. Should any change of grade occur after installation, I will be responsible for adjusting the water line and meter setting to the correct depth as determined by the Water Department.

I understand all charges for the water service materials and correct installation must be paid prior to occupancy of a new residence or business. For an occupied residence or business, all charges must be paid in a timely manner for water service to continue.
