I agree to have water service placed in my (OWNER’s) name after my tenant has asked Leavenworth Waterworks for discontinuance of Service. I further agree to pay all charges for service that may accrue from usage or leaks until the new tenant signs a contract.

• I understand that the provisions of this contract will go into effect automatically after my tenant asks for discontinuance of service.
• I also understand that this agreement does not apply when my tenant vacate the property without notifying Leavenworth Waterworks or service terminated due to nonpayment.

This agreement can be cancelled by Leavenworth Waterworks or myself upon notice by either party to the other party.

**Leavenworth Waterworks requires written notification upon sale of property.
